- The Medical Documentation Services provide the means for the secure electronic exchange of Electronic Clinical records - also known as Electronic health records (EHR), or electronic medical records (EMR) - across different medical stakeholders such as Medical Treatment Facilities to supports continuity of care. It supports the systematized collection of patient and population electronically-stored health information in a digital format. Clinical Records may include a range of data, including demographics, medical history, medication and allergies, immunization status, laboratory test results, radiology images, vital signs, personal statistics like age and weight, and billing information. The minimum standardized data set of medical information must include the data elements relevant for patient regulating.
